Memorial Development Foundation each year.
 Your personal gift to the
Foundation
of $500 or more during the calendar year not only qualifies you
for membership in the
Physicians Circle, but also helps
support the Rockford Health System mission of

Care. Every Day.
For All Our Patients."
Gifts to the Foundation
make a difference.
With your gift, you can help:

support major projects like
facilities renovations and equipment purchases;
- fund training to promote medical
excellence; and

provide care and comfort items to
enhance the experience of our patients and their families.

in the past, and strongly encourage all physicians to consider
charitable giving to the Foundation and membership in the Physicians Circle again this year.
- Total gifts of
$500 or more during a calendar year qualify physicians for
membership in the
Physicians Circle.
- Annual gifts of
$1,000 or more are
eligible for recognition at the Triumvirate Recognition Society
Dinner.

Lifetime total giving of
$10,000 or more
qualifies for Permanent Triumvirate Membership and inclusion of
your name on the beautiful glass donor wall in the hospital
lobby.
