Seven-week series for chronic disease self-management, taking the patient beyond the doctor’s office and into the home
Rockford Health Physicians, the First Presbyterian Church and trained nurse facilitators, in conjunction with the UIC-Extension Office, will present a seveb-week series for those with chronic diseases, targeting symptoms and issues common to many chronic diseases such as fatigue, shortness of breath, emotional/physical/mental stressors, pain, medication management, nutritional issues, sleep problems, anger and depression, enhanced communication with health care providers, etc. This is the original Stanford University evidence-based program for chronic disease self-management.
It will meet in the First Presbyterian Church dining room inside the front door at 406 N. Main, on 7 consecutive Wednesdays, starting on April 29 and finishing on June 10, from 1 to 3:30 PM.
The cost is $25. Advance registration is required with checks made out to First Presbyterian Church/CDSMP and mailed to: First Presbyterian Church, 406 N. Main St., Rockford, IL 61103.
