Rockford Health Physicians is excited to announce the opening of a new medical clinic in Belvidere. The office is located at a highly visible intersection just past the Chrysler plant off US 20 and Belvidere Road.
The facility will be staffed with an internal medicine physician and a pediatrician. It has nine exam rooms and a lab.
Daisy Chacko, M.D., board certified in internal medicine, will join Patricia Moody, M.D., board certified in pediatrics, to form a care team committed to providing respectful care to patients in Belvidere and in Boone County. Both doctors will begin seeing patients on September 17.
Dr. Chacko is new to Rockford Health Physicians, coming from Ottawa, Illinois. Dr. Moody most recently served patients at Rockford Health Physicians Convenient Care on Perryville Road.
